• LIABILITY RELEASE WAIVER:

The client is aware of their own health and physical condition and having knowledge that participation in any exercise program could cause injury. Participation in physical activity through Marvelous Mike and “Marvelous Digital Consulting LLC" is voluntary.

Having such knowledge, the client releases Marvelous Digital Consulting LLC or it’s representatives from liability for accidental injury, illness or death which may incur as result of participating in the physical activity through or outside of Marvelous Digital Consulting LLC.

The Client hereby assumes all risk and responsibility connected therewith and consents to participate in physical activity.
The Client agrees to disclose any physical limitations, disabilities, aliment, or impairments, which may affect my ability to participate in physical activity through or outside of Marvelous Digital Consulting LLC.
The client hereby releases, indemnifies, and holds harmless, Marvelous Digital Consulting LLC or it’s representatives from any and all manner of actions, causes of action, suits, proceeding, debts, dues, contracts, judgements, damages, claims and demands whatsoever in law or equity stemming from the agreement between the below mentioned parties.
Client expressively agrees that this release, waiver, and indemnity clause is intended to be as broad and inclusive, and if any portion thereof is held invalid, it is agreed the balance shall, notwithstanding, continue in full legal force and effect.
This form is an important legal document. It explains the risks you are assuming in beginning a nutrition consulting. It is critical that you read and understand it completely. After you have done so, please print your name, and date below, and
send back. If you agree with all terms and conditions and everything stated in this legal document.
The nutrition advice given by Marvelous Digital Consulting LLC Coaching LLC (which hereafter refers to: Marvelous Digital Consulting LLC) is solely based on the information provided by the Client/individual. The nutrition information given is meant only for the client/individual completing the intake questionnaire form. It is the sole responsibility of the client/individual to provide complete and accurate
information.

Any misinformation, inaccurate or omitted information may affect the nutrition assessment and/ or advice. Any misrepresented information is solely the client’s/ individual’s responsibility.
“Marvelous Digital Consulting LLC” will not be liable. “Marvelous Digital Consulting LLC” provides nutrition counseling and chefs consulting only and is not licensed to prevent, diagnose, alleviate or treat any medical conditions, disease, physical or mental ailments or pain or
infirmities.
Nutrition Waiver and Covenant Not to sue. Have volunteered to participate in a nutrition consulting under the direction of “Marvelous Digital Consulting LLC” which will include but may not be limited to nutrition planning/coaching/consulting/programming. In consideration of “Marvelous Digital Consulting LLC” agreement to assist me, I do here and forever release and discharge and hereby hold
harmless “Marvelous Digital Consulting LLC” and their respective agents, heirs, assigns, contractors, and employees from any and all claims, demands, damages, right of action or causes of action, present or future, arising out of or connected with my participation in any nutrition program including any injuries resulting there from. I acknowledge and agree that no warranties
or representations have been made to me regarding the results I will achieve from this program. I understand that results are individual and may vary.

Nutrition Assumption of Risk
I recognize that specific foods may create allergic and possible fatal reactions, most specifically, products containing nuts. I have therefore specified any food allergies/ sensitivities I am aware of. I am aware that specific foods may interact with
certain medications. I have discussed such food reactions and side effects of all my medications with my doctor or pharmacist and do not hold “Marvelous Digital Consulting LLC” responsible for food and medication reactions.
I also understand the diet plan and/or consulting I receive will not take my medications into consideration. If I am on medication, I am responsible to consult with my doctor before starting a new diet plan/nutrition consulting. If I am pregnant or lactating, have high cholesterol, high blood pressure, high blood sugar, diabetes, renal disease, gastric by-pass surgery, a family history of gout or any other medical condition that requires special dietary restrictions, must receive permission from my physician before participating in the specific nutrition program designed for my use, or may be advised to seek help from
another health professional.

Marvelous Digital Consulting LLC is not a dietician, physician or psychologist, and the scope of his consultation services does not include treatment or diagnosis of specific illnesses or disorders. If you, the client, suspect you may have an ailment or illness that may require medical attention, then you are encouraged to consult with a licensed physician without delay.
While people generally experience greater health and well less as a result of embracing a healthier attitude, lifestyle, and diet, Marvelous Digital Consulting LLC does not promise or guarantee protection from future illness.
By accepting the terms and conditions below below, you acknowledge that you understand that Marvelous Digital Consulting LLC are health consultants and are not physicians, dietitians or psychologists and that you should see a doctor if you think you have a medical condition. Marvelous Digital Consulting LLC will not be held liable for failure to diagnose or treat an illness, nor will he be liable for failure to prevent future illness.

I, the client, read all terms and conditions and agree with everything above.
The Client hereby releases, indemnifies, and hold harmless, Marvelous Digital Consulting LLC from any and all manner of actions, causes of action, suits, proceeding, debts, dues, contracts, judgements, damages, claims and demands whatsoever in law in law or equity. Stemming from the. Agreement between the below mentioned parties.

I have no known physical or mental condition that would impair my capability to participate fully, as intended or expected of
me.
I have carefully read the foregoing release and indemnification clause and understand the contents thereof.
I fully understand the risks of participating in physical activity and agree that Marvelous Digital Consulting LLC shall not be held liable in the case of any training related injury or death.
CLIENT acknowledges before beginning, implementing, or using any and all of recommendations provided, that CLIENT should consult a medical doctor.
CLIENT acknowledges that client is not authorizing to disseminate, share, convey, or transfer any recommendation, either orally or written, provided by Marvelous Digital Consulting LLC without his express written consent.

All payments by CLIENT to Marvelous Digital Consulting LLC Coaching LLC are NON-REFUNDABLE.

Payment is non-refundable once plans are issued due to the custom nature of the work and time put into writing each individualized program. No refunds will be issued during our time together due to the nature of intellectual property provided as well as the 24/7 ongoing support that comes with coaching. Please allow 24-72 hours for plans to be issued after initial purchase.

The contents of the programs are confidential and are only to be utilized by the individual to whom they are addressed. Any attempts to distribute, copy or share the program are forbidden.

All check-ins are responsible to the CLIENT, as new updates and programs updates will only be sent out if adequate information is provided within CLIENT Updates every week. It is the CLIENT's responsibility to follow the protocol provided by Marvelous Digital Consulting LLC.
